A HEALTH and safety trainer from Birmingham has been fined hundreds of pounds - for flouting health and safety regulations which could have led to the death of one of his students One of Steven Roadknight's students suf-f fered rope burns to her hands, a sprained ankle and bruising to her spine after "free falling" three- and-a-half metres from a fork lift truck during a session.

Roadknight, of Gilson Way, Kingshurst, was teaching Monicak Kowalczyk how to evacuate the truck in an emergency when she fell at Sutton Coldfield transport firm Kuehne & Nagel's warehouse in October 2008.
